  • Optional summary sheet of results (ideal for internal records or JCQ inspection).

Assessments are always based on clear evidence of both:

How It Works

  1. School completes Section 1 of Form 8
This includes background, history of need, and confirmation of normal classroom support.

  2. Assessment Day
I assess students using targeted tests based on their needs. These might include:

  • Writing speed (DASH-2)

  • Reading fluency (WIAT-III, ToD-C)

  • Working memory / processing speed (CTOPP-2)

  1. Section 2 Completed
I return Form 8 (Section 2) to the SENCo, alongside any optional documentation.

  2. School submits to JCQ
The SENCo uploads the complete application via Access Arrangements Online.

Please ensure the following are provided on or before the assessment day:

  • Completed Section 1 of Form 8 (with all 3 questions answered)

  • Brief student background (what arrangement is used and why)

  • Clear timetable for each pupil to attend their assessment

  • Confirmation that any recommended arrangement is the student’s normal way of working

  • Any relevant supporting documents (e.g. classroom work samples, SEN records)

⏱ If students are applying for arrangements like extra time, scribe, or reader, their usual classroom support must reflect this (JCQ requirement). Assessment alone is not enough.
